Hole in the Wall is a mission in Fallout 4 and part of IGN's complete Fallout 4 wiki guide and Walthrough. Hole in the Wall takes place in Vault 81 and nets some good rewards, including a new Companion, Curie.
Speaking of Companions, it's a very good idea to have help on this mission if you're low level. Preston Garvey or Codsworth are both solid choices when taking on this quest.
Talk to Overseer McNamara or the medical staff in Vault 81 after completing Here Kitty Kitty and then leave the vault and wait 24 hours to trigger Hole in the Wall. It might not trigger Austin heading to the med bay if you don't leave, wait and return. Once the situation is explained, head to the Reactor room. The junkie will open a hidden pane; and reveal the secret sealed section of the vault.
Inside, you'll be fighting more powerful versions of Molerats. The same rules apply for these enemies as for regular Molerats..they'll pop up next to you. Constantly cycle into VATS to catch them in slow-motion and fell them with head shots.
Two other dangerous enemies await in the ruins of the Vault. Laser Turrets positioned in the ceiling can deal heavy damage to you. Step out from cover with VATS and snipe them systematically to clear the area.
You'll also encounter a Protectron who will lumber toward you. Don't fool around with him..use explosive or energy weapons to kill him quickly. Otherwise exploring the area will be much more difficult.
Beware the Molerat Brood Mother in this den. Kill her minions from a distance and then take her out with your best weapon. Be sure to loot the mini-nuke from inside a downed locker (visually hidden from the door; walk around) in the room the Brood Mother appears out of.
At the end of the path you'll find Curie sealed in a lab. Talk with her through the glass. Tell her you're authorized to open the door and she'll let you in. Grab the antidote and also pick up the Medicine Bobblehead from the nearby desk. If you ask Curie to join you she'll become one of your companions.
Follow the elevator upstairs and loop back to the lab. If you give all of the antidote to the child you'll be rewarded with a Syringer.

Finding The Overseer’s Guardian
Go northwest of Diamond City and get to Vault 81, go inside and take the elevator down to a lower level of the Vault. Remember you’ll need to hand over three power cores to get inside the Vault.
Get out of the elevator, stay on the right hand side and find the Depot room. You’ll find Alexis Combes inside the room holding the Overseer’s Guardian. In order to take the possession of the weapon, you’ll need around 3000 caps.
Lot Of Mods
A lot of modifications can be made to the Overseer’s Guardian. One such potent modification is increasing the damage from a mere 66 to 147 ( You’ll need to have Gun Nut Rank 3 Perk). When you are in Level 39 and reach Gun Nut Rank 4 you can increase the damage of the gun further to 162.

What makes Overseer’s Guardian So Powerful?
One of the main reasons for liking the rifle is flexibility. It has an ability to be turned into a short, medium or long range weapon according to your needs and preferences.
The rifle works on both fronts and can kill the enemies quietly, without getting noticed once you put a suppressor in it. The damage and accuracy of the gun is also good enough and there lot of modification options available in case you want to adjust it.

Spoilers for one of Fallout 4’s vaults ahead!
In the Fallout universe, cruelty and Vault-Tec seem to go hand in hand. Those lead-lined safe havens scattered around the country serve as petri dishes for whatever sick experiments the higher-ups in the company have in mind. Sometimes these can be as mild as testing cryogenic sleep (without permission from the test subjects, of course), or as extreme as forcing people into what are essentially death camps.
Vault 81, which can be found west of Diamond City, is a Vault that falls nearer to the more extreme end of the Spectrum.
The Vault’s main experiment was to test the effects of different diseases and antibodies on subjects under the effects of extreme radiation. Diseases were to be pumped into the rooms as they were monitored by a set of scientists who seemed to enjoy this all a bit too much.
The Secret Vault 81 (located under the already demented Vaulted 81) was used to breed horribly mutated mole rats, as if the ones on the surface weren’t enough. These are a bit nastier, with the ability to transmit diseases to their victims. While it can be argued that most of the experiments, no matter how insanely unethical they were, could have served some sort of purpose for humanity, these things have no reason to be alive.
Now, you’re probably wondering how this vault was successful. After all, they were not only dosing an entire population with infectious diseases, they were also going to essentially give them all cancer and cause horrible mutations. Well, you see, the experiments never actually ended up happening thanks to one individual.
Each and every Vault has an Overseer. The one person who knows everything that’s going on and usually has a moral compass that always points to hell with zero deviation. They keep everyone in line and are the only ones with contact to Vault-Tec (usually). They are hand-selected to be ruthless, to feel nothing, and to do as told. Unfortunately for Vault-Tec, the initial option for the Overseer was no longer able to fill the position so a second option was made.
This man was smart, understood how things worked, was fully qualified, but wasn’t trying to kill people for “science.” He decided to take matters into his own hands and it all began with him severing all communication to the outside world after receiving word of of the impending nuclear attack. Gripped by the shame of denying a direct order and separating the people who trust him from the world, he decided to err on the side of humanity.
Because of that one Overseer’s decision, the inhabitants of Vault 81 never died. In fact, it prospered so much that at the time of Fallout 4’s main adventure, 200+ years later, the community still thrives. The Vault opened its door and let in the world.
Outsiders trade, all of the facilities work as they should, children are born. Life is being lived in a Vault after the Great War without murder, stealing, blackmail, or hatred. It is an aspect of civilization that has managed to remain true to itself, despite the adversity it faced. It’s just humanity.
